Mankato Clinic offers Team-based Care. This means you, as a patient, will have a care team and a more personalized visit with your healthcare provider. Your appointments will have a Patient Care Coordinator joining you and your provider in the exam room. Between visits, you have the support of a care team.

What can I expect at my visit?
During your appointment, your Patient Care Coordinator will join your provider in the exam room. Using a laptop computer, a Patient Care Coordinator documents what is discussed in the appointment. This frees up your provider to focus on you during the visit. As always, you can have a one-on-one conversation and exam with your provider.
Who is on my care team?
Most care teams are made up of a provider and two Patient Care Coordinators who are nurses or medical assistants.
What can I expect from my care team? Your care team is a team of skilled healthcare professionals who work together to meet your needs. When follow-up care is needed, your Patient Care Coordinator can help you set this up and update all providers involved in your care.
How will Team-Based Care improve my care?
A care team supports your care from preventive health screenings to managing chronic conditions. Patient Care Coordinators also help improve communication between patients and providers because you may call your care team directly and leave a message for a response.
How do I contact my care team?
Each care team has a phone number where you can call and leave a message. Or you can email your provider through the patient portal. Your Patient Care Coordinator may also ask you how you prefer to communicate – phone calls or emails through the patient portal. If you don’t have a patient portal account, your Patient Care Coordinator can help you set up your account.
How do I make appointments?
